Introduction and background
Community Connections is a social prescribing service focused on supporting isolated adults in the London Borough of Lewisham. The Community Facilitators (CFs) arrange home or phone appointments with people to discuss their needs during a Person-Centred Plan (PCP). Based on what the person feels are the challenges to accessing the community, the CF will signpost and refer them to support services and social groups, such as: transport schemes; meditation groups; Occupational Therapy; the list goes on!
The CF is then in regular contact with a Community Development Worker (CDW) so that together they can look at where there are gaps in provision and the CDW can look to build capacity within the Voluntary Sector.
